To successfully file a counter notice regarding a copyright infringement claim, you must provide the following information:

• Your name, physical address, and email address;
• A detailed description of the material that has been reported as infringing;
• Information sufficient to identify the original material (e.g., the specific URL where the original content is located);
• A statement asserting that you have a good faith belief that the material was removed or disabled due to a mistake or misidentification and that the information provided in this counter notice is truthful, accurate, and complete to the best of your knowledge;
• An electronic or physical signature of the person submitting the counter notice.
