- Information sufficient to locate the allegedly infringing material (e.g., the specific URL where the content is located);
- A statement asserting that the complainant has a good faith belief that the material's use, as described in the complaint, is not authorized by the copyright owner or their representative and that the information provided in this report is truthful, accurate, and complete to the best of the complainant's knowledge and belief;
- An electronic or physical signature of the copyright owner or person authorized to act on behalf of the copyright owner.

To successfully file a counter notice regarding a copyright infringement claim, you must provide the following information:
- Your name, physical address, and email address;
- A detailed description of the material that has been reported as infringing;
- Information sufficient to identify the original material (e.g., the specific URL where the original content is located);
- A statement asserting that you have a good faith belief that the material was removed or disabled due to a mistake or misidentification and that the information provided in this counter notice is truthful, accurate, and complete to the best of your knowledge;
- An electronic or physical signature of the person submitting the counter notice.
